• 12-17 years old;

• Willing and able to participate in all activities and comply with program rules;

• Not actively suicidal, assaultive, or destructive and do not display any symptoms of psychosis; 

• A current (ASAM) assessment indicating the need for residential treatment.

 

 

Required Documents
Once you have scheduled an admissions date with our intake coordinator, please feel free to print, complete, and FAX or email back these documents to the Residential Admissions Department.  However, this is not required and can be completed during the intake process.
